02 · TITRATION
Weeks 1-4. Slow start.
Semaglutide 0.25 → 0.5 mg, or Tirzepatide 2.5 → 5 mg. Weekly.
Most side effects (nausea, constipation, fatigue) happen during titration. Push through with anti-nausea management, not by skipping doses or speeding the ladder.
- Hydration 2.5L+ daily
- Fiber 30g+ daily
- Anti-nausea: ginger, B6, peppermint
- Stop at the lowest dose that produces appetite suppression
03 · ACTIVE LOSS
Weeks 5-10. Maximum effective dose.
Hold at the dose that produces ~1-2 lb/week loss.
Protein at 1g per pound of lean mass. This is the most important variable. GLP-1s suppress appetite, including for protein — actively prioritize protein at every meal.
Resistance training 3-4x/week. Non-negotiable. Without it, ~30% of weight lost is muscle. With it, ~10-15% is muscle.

04 · OFFRAMP
Weeks 11-12 and beyond.
Hold at peak for 3-6 months, then taper.
Don't stop suddenly. Appetite rebound + lower resting metabolic rate = weight regain.
Standard taper: hold at peak dose until goal hit + 3 months. Then step down one rung every 4-8 weeks. Hold at each step until weight stable 4 weeks.
